For Professionals and Community Partners

1. Speak with the family. Discuss the referral and obtain the appropriate permission before sharing their information.

2. Connect with our team. Call us or send a general inquiry with your own contact details and a request for a referral discussion.

3. Confirm next steps. We will discuss service-area coverage, availability, insurance requirements, and how to provide any necessary documents through the appropriate channel.

Please do not include a child’s name, diagnostic report, medical history, Social Security number, or insurance documents in the general website contact form. Call for instructions before sending private information.

What Happens After a Referral?

We discuss the family’s priorities, review relevant requirements and documentation, and determine the appropriate assessment and next steps. A referral or completed intake form does not guarantee eligibility, insurance approval, an available appointment, or the start of services.

Office hours: Monday–Friday, 8:30 AM–5:00 PM. Saturday appointments by request.
